1.  Family Life Enrichment Centers, Inc.:  a 100-bed skilled nursing facility with rehabilitative services;

2.  Portico Place Manor:  an Independent living facility with supportive services;

3.  Georgia Home Health Services, Inc.:  a certified home health agency providing skilled nursing, physical and occupational therapies in the home;

4.  Family Life Homecare, Inc.:  a licensed private duty sitter/companion agency providing hourly care in the home;

5.  CIR/Family Life Medical Supply, Inc.: a medical supply company.
